This contract is entered in this day of __________ between Bill Meyer dba Carisbrooke Equine Transport, Stillwater, MN (transporter) and ___________________________ (Owner) for the transportation of ___horse(s) and some equipment from ______________________________to ____________________________________, for the price of ___________________________.
1. Pick-up and Delivery:
Transporter shall pick up horses and equipment at the facility located at ____________________, and shall transport them directly to _______________________.
2. Travel Time:
The distance is approximately __________ miles. It will take approximately ____ days to make the trip. Federal transportation law allows a driver 11 hours of on duty time per day, which translates to approximately 600 miles per day. Transporter stops approximately every 2 hours to allow the horses to relax and relieve themselves, and each night for 8 to 10 hours. The horses have feed and water available 24 hours per day. The horses are not removed from the trailer or placed in lay-over facilities at any time or transferred to other transporters unless by prior arrangement with Owner.
3. Fees and Payments:
Owner shall mail a check, make a wire transfer or a credit card payment through PayPal, in the amount of ½ of the transportation charge of;_____________ to Transporter by ______________________, and shall provide Transporter with cash, check, wire transfer or PayPal payment for the remaining amount upon pick up or delivery of the horses, depending on the location of Owner. Owner will also be responsible for supplemental charges such as emergency medical charges which may be incurred during the transport of the horse(s)
4. Insurance:
It is the responsibility of Owner to provide insurance (mortality and/or major medical) for the horses. Transporter does not provide insurance coverage other than gross negligence coverage for horses in transit.
5. Owners requirements:
Owners must provide original coggins test documents, or faxed copies from the testing laboratory if there is not time for the documents to reach the horses prior to transport, (coggins must be dated within 6 months of travel date to be current in Ca, Co and Canada ), and a current health certificate for interstate transport (current is within 30 days of travel date, or an international health certificate if transporting across international borders). These documents must be with the horses at the time they are picked up. If a horse is being permanently transferred to Canada, a Goods and Services Tax (GST) of 7% must be paid to Transporter, as this fee must be paid at the time of crossing. If permanently entering the US from Canada, there may be up to a $160.00 brokerage fee, which must be paid to Transporter in addition to the transport fees to cover the brokerage costs charged at US Customs. In addition, for horses crossing international borders, Owner must also provide a Power of Attorney letter authorizing Transporter to act a Authorized Agent for Owner in paying any taxes, duties or charges incurred by the horses care during its transport. This letter authorizes Bill Meyer dba as Carisbrooke Equine Transport to act as my authorized agent in all matters relating to the transportation, care, custody and control of the ____ animals being transported, including the payment of all taxes, import or export duties or other charges including veterinary charges that may be incurred on my behalf, and which I agree to reimburse Carisbrooke Equine Transport for upon delivery of the animals.
